A Clinical Study on Facial Bone Fractures of Korean


Abstract
The author studied 1488 patients who were admitted and treated in Severance Hospital of Medical and Dental College of Yonsei University from Jan. 1982 to Dec. 1992 for facial bone fractures.
@ES The result obtained are as follows:
@EN 1. The most frequent causes were automobile accident(39.7%) and fight(25.6%).
2. Facial bone fractures occurred four times more frequently in male, and the incidence has been incressing by 150% in the last 5 years.
3. Facial bone fractures were most frequent in September and least frequent in January, and steadily increased toward summer and decreased thereafter. Facial bone fractures were most frequent in the 20's.
4. Facial bone fractures occurred most frequently between 18 to 24 o'clock.
5. Time elapsed between the injury and treatment was less than 24 hours in 72.4%, and 83.6% of subject received treatment within 3 days.
6. mandible was the most frequent site(47.5%), and zygoma & zygomatic arch(15.2%), maxilla(14.5%). Nasal bone(8.1%), and others(6.6%) follow. Simple fractures comprised 85.1% while compound fractures occured in 14.9% of subjects. Number of
fracture
sites were : 1 site(85.1%), 2 sites(8.6%), 3 sites(5.2%), and more sites(1%).
7. Mandible fracture occurred as isolated fracture in 87.5%, while other fractures occurred concurrently in 12.5%. Average number of fracture sites was 1.7. Frequent fracture sites were : symphysis 37.6% and angle 21.2%. Left side fractured more
frequently by 1.7times. Simple fractures were most frequent(53.8%), while tooth and alveolar bone injury comprised 38.5%. The most frequent type of treatment rendered was : open reduction and closed reduction combined(62.4%), and the most
frequent
method of fixation was interosseous wiring(49.1%), and the rigid fixation is in increasing trend.
8. In maxilla, simple fracture occurred in 36.6% and compound fractures(63.4%), and the most frequent type of fracture was Le Fort II(48.2%) : unilateral fracture(67.3%), and concurrent teeth and alveolar bone injury comprised 39.8%. The most
frequent
type of treatment rendered was open reduction(42.7%) and the most frequent method of fixation was interosseous wiring(67.6%) with rigid fixation in increasing trend.
9. In zygoma & zygomatic arch, simple fractures occurred in 51.1%, compound fractures 46.9%, and the most frequent sites were : zygoma & zygomatic arch(43.9%), zygoma(37.2%), and zygomatic arch(19%).
Left side was more frequent, with the left zygoma & zygomatic arch being the most frequent site(23.3%).
Teeth and alveolar bone injury occurred in 27.2%. Open reduction(41.9%) and observation(26.7%) were the most frequent types of treatment rendered. The most requent method of fixation was interosseous wiring.
10. In nasal bone fractures, simple fracture occurred in 552% while compound fracture were 44.8%. Treatment rendered were : observation(57.3%), closed reduction(36.8%), being the most frequent.
11. The average number of associated injured sites in the rest of the body was 0.8, with the most frequent areas being : facial contusion and laceration(34.7%), head(27.4%) and extremity(21.0%).
The number of the sites involved were 1 site in 55%, 2 sites in 31% and 3sites in 14%.
12. Complications during the admission occurred in 104cases(7.0%), with infection(43%), neurologic complication(28%), and others(16%), be9ng the most frequent causes.
